Name Changes

A person who wishes to attain a legal name change may file an application with the probate court where the person wishing to change names resides.

Adult Name Change

If you want to change your name, you must be 18 or older and a resident of Jefferson County for at least 60 (sixty) days prior to the filing the application.

Minor Name Change

If you want to change the name a person less than 18 years old, you and the minor, must be a resident of Jefferson County for at least 60 (sixty) days.
Only one of the natural parents, a legal guardian, or a guardian ad litem should fill out the application.
You must appear at the hearing and speak with the Judge.
Once the Court has approved the application, the name change will be granted.  You must change your name on official documents, but you cannot change the original birth certificate.

The name change filing fee is $150.00
